About Case Management
Case management provides help to families, kinship caregivers and individuals by providing advocacy and support, connection to resources in the community, referrals for legal assistance, and other services. Services are designed for families with at least one child residing in the home or individuals who have experienced out-of-home placement. Eligible clients should also live within a 45-mile radius of an MCH Family Outreach office.
The program provides a combination of individual and family services, case management, and advocacy to help the family meet its needs. We help families gain access to community resources that will help their children grow to be healthy and successful at home, at school and in the community.
MCH Family Outreach case managers work with individuals and other people in your family and neighborhood who are important to you. We work with you to identify and build on your family’s strengths to help you meet your needs and those of your children. We meet with you in your home, your child’s school, or anywhere in the community where you are comfortable…as often as needed.
All case management services are confidential and free to eligible families and individuals.
